Basis – Ulnaropathie Protocol Echografie

Echo is bij ulnaropathie complementair aan EMG, helpt bij lokaliseren (ontbreken geleidingsstoornissen of uitsluitend axonale schade) en vaststellen van de aard van de afwijking (tumor, cyste, relatie met snapping triceps of callus vorming, etc).(1-3)

Meting:

• Zenuwdikte (CSA) thv cubitale tunnel (midsulcus), distale en proximale sulcus, 1/2 bovenarm.(1, 2)
• Aanwezigheid van hypervascularisatie.(4)
• Scannen gehele arm (onderarm, Guyon) en beoordeling op anatomische varianten (luxatie, snapping triceps), callus vorming, etc.

Voor ulnaris (druk) neuropathie thv elleboog (UNE) kunnen de metingen beperkt blijven tot elleboog en bovenarm, waarbij proximale en distale segmenten van ulnaris wel gescreend worden op (andere) focale afwijkingen; bij vraagstelling ulnaropathie thv Guyon kunnen parallel aan CTS de metingen tot het pols en onderarm beperkt blijven met behoud van screening proximale segmenten (eveneens op andere focale afwijkingen, indien de klinische lokalisatie toch onverhoopt onjuist zou zijn).
